There is no form, and a one-line message is a good message. The reply comes from me. If you want a specific reply rather than a polite one, three things do most of the work.

What you are trying to do, in your own words.
- The ground
What has already been tried, and where it stopped working.
- The outcome
What a good result looks like, and by when it has to exist.
